Cost

Certified copies are available to entitled individuals for a fee of $15.00 per copy.

Non-certified copies are available for $5.00 per copy.

Payment may be made in person by cash, check, or credit/debit card. For requests submitted by mail, please remit payment by check or money order made payable to the Boone County Recorder.

Available Records

Birth Records

Iowa birth records from 1984 to present are available in all counties statewide, regardless of the county in which the birth occurred. Birth records dated prior to 1984 must be obtained from the county of birth or from the Iowa Department of Health and Human Services, Bureau of Vital Records.

In Boone County, birth records are available beginning in 1880. These records exclude out-of-wedlock births prior to July 1, 1995, fetal death records, and adoption records.

Marriage Records

Certified copies of marriage certificates are available for marriage records from 1954 to current and are available in all Iowa counties, regardless of where the marriage occurred.

Boone County also maintains limited marriage records dating back to 1851. These older records contain minimal information and are available through the present.

Death Records

Statewide death certificates from 1954 to current are available in all Iowa counties, regardless of the county of death. Boone County death records begin in 1880 and continue to the present.

Instructions

To obtain a Certified Copy of a Vital Record, you must be entitled to it.  Entitlement means the person named on the record is yourself, a legal parent, grandparent, spouse, brother, sister, child, legal guardian or legal representative.  You will need a valid photo ID.

Ways to Apply

  • Apply in person.
      • Make sure to bring your valid ID or driver’s license and a check or money order (payable to Boone County Recorder), cash ($15.00 per copy) or credit/debit card for payment.
  • Applying by mail.
    • If applying by mail, you must fill out the application with your signature notarized, and we need a copy of your valid ID or driver's license and a check or money order (payable to the Boone County Recorder).
      • If you would like Boone County to process your request, please mail your notarized completed application and payment to:
